Cancellations
No Refund
| · | I appreciate your business, but if for any reason you need to cancel or reschedule an appointment please allow a 24-hour notice. Regretfully, any late cancellations with less than 24 hours notice will be billed for the full scheduled service amount. |
| · | No-shows will be billed for the full amount of the scheduled procedure(s) on your charge card. NO EXCEPTIONS! |

| · | Payment in full is expected at end of service(s). |
| · | I accept MasterCard, Visa, Discover and American Express, personal checks, and cash. |
| · | Services paid by credit card will incur a 4 % processing fee of services. |
| · | Returned checks or declined credit cards will incur a $35 Service Charge |
Etiquette & Courtesy
| · | In consideration of all guests, we ask that you either turn off pagers and cell phones or use the vibrate setting when you are visiting Permanent Great Looks. |
| · | Tipping is left to your discretion. If you are pleased with your service it is customary to extend a 15% to 20% gratuity. |
| · | Rates are subject to change without notice. |
| · | Discounts, coupons and other price adjustments cannot be combined. |
| · | Please note a major credit card will be required to hold your appointment. Your card will not be billed and you may pay for your services by other means, but card will be charged if you do not follow cancellation policies. |
| · | Please be on time for your appointment, so you can enjoy it, and not feel rushed. Please be considerate of those that may be scheduled after you. Appointments cannot be guaranteed for late arrivals. |
| · | No Children under the age of 18 allowed during permanent makeup procedures. |
